To Delete a Provider Type from the list, click the checkbox to the left of any Provider Type, then click Delete at the top of the Provider Type List window to finalize the deletion, or click Cancel if you change your mind.

It is not necessary to delete Provider Types already in the system by default. You can simply choose not to use them.
