Small-Group Doctors: Call Schedule Sharing

With three general steps, groups can benefit from call-sharing just like bigger practices.

Physicians are feeling the squeeze these days: decreasing numbers of available on-call physicians, increasing mergers, on-call compensation issues, and the need to provide 24/7 on-call coverage is taking its toll on physician morale no matter the size of the group. The increasing use of hospitalists and technology such as EHR has great potential to alleviate stress for physicians in medium and large groups, but what steps can “smaller” practices take? Here are some ideas I have learned while training smaller on-call scheduling groups.

Who Is Your On Call Infectious Disease Physician?

In light of the current Ebola issues, and previous brushes with SARS, H1N1, Anthrax, and MRSA concerns, shouldn’t your Group consider having a designated Infectious Disease physician on call? What would it take to assemble an ID schedule if you had to?

Do you know who the individual Infectious Disease specialist is vs a general Internal Medicine number to “dial and pray”?

How long would it take you to find out who the appropriate individual doctor is?

On Call Schedule: When equal is not fair!

So, you're ready to get moving on your next schedule, eh?

   1). You have had "the meeting" and know who is covering the Holidays. Check.

   2). You know who is unavailable due to vacations, CME or other things that take them out of call, and have those requests assigned. Check.

   3). Now, how about those weekends?

I have had several calls lately where weekends no longer need to be "fair", or are fair in some way other than being equal in the tallies.
